Intake Form Question Fields

When building an intake form template in Clio Grow, you can add, edit, remove, and manipulate new questions relevant to your form template. You can also link your custom fields to some question types.

Add questions

Intake form questions can be added in the form of single-line text, paragraph text, multiple choice, multi-select, dropdown, file attachment, date, or yes/no checkbox. You can also add section breaks to your form with additional instructions.

Note: The maximum file size for a file attachment is 50MB.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. From the field choices menu on the left, select a question type listed under Add Fields.
  3. Enter the question details.
  4. Click Save Question.

 

Edit questions

You can edit all intake form question types to add or remove information. You can also make questions required, add descriptions to questions, and link some question types to custom fields

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Choose one question you want to edit.
  3. Click Edit on top right of question box.
  4. Enter new question details.
  5. Click Save Question.

 

Add options to questions

When creating or editing a question type, you can add additional options or selections to some question types. This way, your prospective client can choose one of the selections that you listed when completing the intake form. 

Note: This feature is only available for multiple choice, multi-select, and dropdown questions.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Add a new multiple choice, multi-select, or dropdown question or click Edit on any existing multiple choice, multi-select, or multiple choice question.
  3. Enter the question details.
  4. Click Add an option.
  5. Enter the first option.
  6. Optional: Click Add a followup to add a followup question to a yes/no or multiple choice question field. Learn more below.
  7. Repeat steps 3-4 for additional options.
  8. Click Save Question to finish.

 

Add followup questions

When creating or editing a question type, you can add followup questions to some question types. With followup questions, you can ask your prospective clients to answer additional questions based on a selection they made for a specific question type.

Note: This feature is only available for yes/no checkbox and multiple choice questions.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Add a new yes/no checkbox or multiple choice question or click Edit on any existing yes/no checkbox or multiple choice question.
  3. Next to an option, click Add followup.
  4. Select one of three question type fields for the followup and enter the followup question.
  5. Click Save Question to finish.

 

Duplicate questions

When duplicating a question, all components of the question will copy to the duplicate other than the custom field connection. This means that the duplicated question will not retain a connection to a custom field if you connected the original question to a custom field.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Click Duplicate on a question to make a copy.
  3. If necessary, you can modify a question.
  4. Click Save Question.

 

Delete questions

If you no longer need a question, you can delete it from your form template. A deleted question cannot be recovered or saved anywhere.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Select a question and click Edit.
  3. Click Delete Question.
  4. Select Delete Question to confirm the action.

 

Make questions required

If you make a question required on an intake form, your prospective clients will be required to answer the question before submitting the form.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Add a question to the form or edit an existing question.
  3. Click Question settings.
  4. Check the Required question checkbox

 

Add descriptions in questions

If you need to provide an example answer input, define a term, or otherwise explain your question in greater detail, you can add some explanatory text to a question.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Add a question to the form or edit an existing question.
  3. Click Question settings.
  4. Add a description in the Question description box.
  5. Click Save settings.
  6. Click Save Question.

 

Change the order of questions

You can change the order of your question boxes in two ways—by manually changing the number of each question box or by dragging a question before or after another.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Select a question.
  3. Click the grayed number on the upper left of the question block.
  4. Enter the new number to change the position.

 

Link questions to custom fields

Once you have created custom fields in Clio Grow, or synced your custom fields from Clio Manage to Clio Grow, you can link question types in your intake form to matching custom fields. Custom fields will only appear as selectable options in question types if the custom field type is a match to the question type.

The table below shows the custom field types that connect to each question type. Follow the numbered steps below the table to learn how to link questions in your form to custom fields.

Picklist/dropdown Dropdown

Note: When you link a picklist/dropdown custom field to a dropdown question, the dropdown question will automatically populate with your custom field picklist/dropdown options and override any existing dropdown options you may have added to the question.

  1. Create a new intake form or edit an existing one. Learn more about creating and editing intake forms here.
  2. Click Edit on a question.
  3. Click Question settings.
  4. Check the box for Connect to custom field.
  5. Choose whether you want to connect to a contact or matter custom field and then select the relevant custom field from the dropdown options.
    • If you have multiple contacts on the form, choose the contact for the custom field connection and then choose the relevant custom field from the dropdown list.
  6. Click Save settings.
  7. Click Save question to save the question..
Note: You cannot select custom fields for the following question fields in the intake form builder: paragraph text, multi-select, section break, or file attachment.
